Responsibilities

To provide direct care and services to clients on the MR/DD Unit under the supervision of the QMRP and the RN/LPN Supervisor. Provide assistance with activities of daily living and completion of habilitation programs. Maintain healthy, physical, and psychosocial environment for the clients.

 

A. Client Care Functions

 

Duties:

 

  • Participate in client care conferences and other facility meetings as assigned.
  • Conduct client programs as instructed by the QMRP.
  • Listen to client and family concerns and report concerns to the RN/LPN Supervisor or QMRP, as appropriate.
  • Assist clients with the activities of daily living, including bathing, dressing, grooming, toileting, mouth care, nail care, and positioning and transferring in and out of chair, bed, etc.
  • Assist with client activity programs.
  • Report skin problems and any other changes in client condition to the RN/LPN Supervisor and QMRP.
  • Transport clients to the dining room and activity programs.
  • Provide active and passive range of motion as necessary
  • Perform vitals.
  • Provide restorative nursing programs as directed by the RN/LPN Supervisor, QMRP, or the Restorative Nursing Supervisor in accordance with the IHP.
  • Read and follow daily care plans and document care provided as required.
  • Provide encouragement, love, and emotional support to clients.
  • Promote client independence by allowing the client the opportunity and time to complete the activities of daily living.
  • Answer call lights.
  • Prepare clients for discharge, transfer, or workshop.
  • Provide post mortem care.
  • Make beds.
  • Straighten clients' drawers and closets.
  • Clean wheelchairs, geri-chairs, walkers, etc.
  • Work with other members of the client care team to provide quality client care.
  • Solicit advice and guidance from other members of the client care team related to the provision of client care and programs, as needed.

B. Safety and Infection Control Functions

 

Duties:

 

  • Understand and follow safety and sanitation rules.
  • Promptly report all violations to the RN/LPN Supervisor or QMRP.
  • Report client accidents and incidents to the RN/LPN Supervisor on the shift in which the incident occurred.
  • Understand and practice infection control procedures, including universal precautions.
  • Dispose of soiled or infectious materials in an appropriate manner.
  • Attend all mandatory inservices including, but not limited to, safety, infection control, OSHA, and the Corporate Compliance Program.
  • Report immediately any hazardous our unsafe equipment or supplies.
  • Assist in maintaining a safe and secure environment for clients, staff, and guests.

C. Food Service Functions

 

Duties:

 

  • Assist clients with dining, as needed, and record food and fluid intake.
  • Pass snacks, reporting percentage consumed.
  • Prepare clients for meals and snacks.
  • Assist clients in meal preparation.
  • Assist with meal service delivery and clean-up.
  • Identify food arrangement and assist clients with dining, as needed.
  • Assist clients with menu selection.
  • Ensure that food is of an appropriate temperate, appropriate texture, and the correct diet prior to consumption by the client.
  • Assist in creating a dining atmosphere that is conducive for an enjoyable dining experience.
  • Record food and fluid intake.
  • Perform after-meal client care.

D. Clients' Rights Functions

 

Duties:

 

  • Comply with the Bill of Rights for Persons with Mental Retardation/Developmental Disabilities.
  • Promote positive relationships with clients, families, and regulators, including a professional appearance and attitude.
  • Maintain client confidentiality and privacy.
  • Treat clients with kindness, dignity, and respect.
  • Report all client complaints and accidents to the RN/LPN Supervisor or QMRP.
  • Report immediately all violations of Clients' Rights.

E. Marketing Functions

 

Duties

 

  • Welcome new clients and their families, new employees, and guests.
  • Promote the services offered by the MR/DD Unit to clients, clients' families, staff, and members of the community.

F. HIPPA Compliance Functions

 

Duties:

 

  • Maintain the confidentiality and security of all PHI, as defined by HIPPA, unless the client or employee consents and authorizes the use or disclosure of PHI or that which is permitted by the HIPPA Privacy Standard. This includes healthcare information, oral or written, which is individual ID information relating to a client's or employee's health and healthcare needs or payment for healthcare.

All other Duties as Assigned.

Qualifications

High School graduate or equivalent required.

  • Must meet all federal, state, and local health regulations, and pass a post-employment offer physical exam. This requirement includes drug screening, criminal background investigation, and reference inquiries.
  • Must be capable of regular attendance.
  • Must have the ability to read and write in English and follow oral/written directions at a level necessary to perform the job.
  • Must have a sincere caring for and interest in people with MR/DD.
  • Must have an ability to relate positively to clients, their families, and to work cooperatively with other employees.
  • Must be able to perform the essential functions of the job with or without accommodation.

Physical and Sensory Requirements (With or without the aid of mechanical devices)

 

Walking, reaching, and bending. Grasping, pushing, pulling, and lifting a minimum of 50 pounds. Fine hand-motor coordination. Ability to distinguish smells, tastes, and temperatures. Ability to hear and respond to client requests. Ability to stand for long periods of time. Ability to read and write in English. Ability to identify potential hazardous conditions by sight. Ability to write or record intake. Ability to communicate with clients, families, and personnel and support agencies. Ability to remain calm under stress. Ability to manage multiple priorities simultaneously. Ability to make decisions. Ability to understand and apply training and in-service education. Ability to maintain confidentiality of sensitive information.
