St. Mary of the Woods, part of the Atrium Centers, Inc. family, has an immediate opportunity for a full time Licensed Social Worker at our campus setting that includes Independent Living, Assisted Living, and our five star rated Skilled Nursing with transitional care rehabilitation operating in the heart of Avon, Ohio.

The Licensed Social Worker must have experience in discharge planning in a fast-paced transitional care - skilled nursing unit and have strong relationships with DME providers as well as hospice and home health care providers. This position is responsible for developing, planning, implementing, and evaluating social service programs and services in accordance with state and federal regulations. This position will identify and provide for residents’ social, emotional, and psychological needs and aid in the development of the resident’s full potential. This position will provide resident's service coordination within the facility and externally and perform all duties using independent judgment and discretion to implement regulations and policy. This position coordinates with the marketing team to effectively manage the admissions process and maintain the positive image of the facility. In addition, this position will be part of the back-up team for the Admissions Coordinator.
•Must be Licensed Social Worker.
•Must, at minimum, have a bachelor’s degree in a human service field and one year of supervised social work experience in a health care setting – preferably with the psychosocial aspects of the elderly.
•Must possess a genuine caring for and interest in the elderly and physically and at times residents with cognitive impairment.
•Must be able to relate positively to residents, their families, employees, and the community at large.
•Must be able to communicate effectively.
•Working knowledge of nursing home regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid.
•Must meet all federal, state and local health regulations and pass a post-employment-offer physical exam. This requirement includes drug screening, criminal background investigation, and reference inquiries.
•Must have the ability to read and write English.
•Must be able to work holidays, evenings, weekends as needed and on rotation as well as Manager on Duty rotations.
•Must be able to assist residents on the entire campus (Independent Living, Assisted Living and Skilled Nursing)
