    • The Social Work/Admissions Director is responsible for developing, planning, implementing, and evaluating social service programs and services in accordance with state and federal regulations.  To identify and provide for residents’ social, emotional, and psychological needs.  To aid in the development of the resident’s full potential.  To assist in the planning for the resident’s discharge.  To provide service coordination. 


      The Social Work/Admissions Director is also responsible to provide family counseling, as needed.  Performs all duties using independent judgment and discretion to implement regulations and policy.


      The Admissions duties include:

      • Develops and executes tactics to maximize admissions of residents requiring a sub-acute level of care including regular contact with potential referral sources including hospitals, physicians, insurers, case management companies and health care agencies.
      • Maintains an 80% conversion ratio from referrals to admissions.
      • Ensures admission screening of potential residents, determines level of care, services required, equipment needs and insurance coverage, as appropriate.
      • Ensures company processes are followed to ensure the clinical and financial approval process in a timely manner.
      • Coordinates admissions with appropriate departments and staff.
      • Directs efforts to the managed care industry by setting up appointments with case managers and explaining the services provided.
      • Contacts local community agencies to generate a positive image and encourage word-of-mouth referral activities.
      • May be responsible for recruiting, hiring, providing orientation/training, and retaining a sufficient number of qualified staff to carry out Admissions Office responsibilities.
      • Performs other duties as assigned.



    •Licensed Social Worker preferred.
    •Must, at minimum, have an associate's degree in a human service field and one year of supervised social work experience in a health care setting – preferably with the psychosocial aspects of the elderly.

    •Must possess a genuine caring for the elderly and physically and mentally challenged people.
    •Must be able to relate positively to residents, their families, employees, and the community at large.
    •Must be able to communicate effectively.
    •Working knowledge of nursing home regulations, including MDS procedures, and reimbursement and eligibility guidelines under Medicare and Medicaid.
    •Must meet all federal, state and local health regulations and pass a post-employment-offer physical exam.  This requirement includes drug screening, criminal background investigation, and reference inquiries.
    •Must have the ability to read and write English.
    •Must be capable of performing the Essential Job Functions of this job with or without reasonable accommodation.

    • Must be proficient with Microsoft Office products (Word, Excel, PowerPoint).
